Eschalon Assessment Which of the following lab equipment can you operate?
Do you posess an allergy or aversion to Dihydrogen Monoxide?
Have you been exposed to Trypanosoma brucei, Encephalitis, Lysosome damage, or Dysarthria?
Do you have any medical conditions that would preclude you from running, jumping, swinging, grappling, fencing, or any other strenuous activity?

Are you afraid of the dark, loud noises, velociraptors or para-biological entities?


Waiver and Release BioCoreTek requires able-bodied persons to assist with our extraction and eradication efforts. The candidate recognizes that the work conditions they may encounter will be variable. The candidate also recognizes that their job duties may change without notice due to hazards and other unpredictable events that may arise (hence the 'Rapid Response' designation).

From time to time, BioCoreTek conducts research into cutting edge fields that may or may not have any documented restrictions or government regulations. This may lead to working conditions that do not adhere to standard workplace safety and hazard requirements.

Armament training may be required onsite. Advanced training in any paramilitary or other martial art is not required, but definitely an asset.


1. I understand the nature of the activities that I will engage in as described above. I understand there are risks of injury and death associated with these activities. I acknowledge and voluntarily assume the risks of illness, injury, and death associated with these activities, inherent and otherwise, and whether or not described above, including those which may result from the negligent acts or omissions of other participants or staff.(__________)

2. I hereby release, indemnify, and hold harmless BioCoreTek, its owners, agents, and employees, and the owner or owners of the lab in which I will be stationed (the Released Parties) to, and agree not to sue them for, any liability for causes of action, claims and demands of any kind and nature whatsoever that may arise out of or relate in any way to my application or participation in BioCoreTek's programs. The claims hereby released and indemnified include, among others, claims of other participants and of members of Participant’s family and claims of negligence of a released party, but not the claims of gross negligence or willful injury.(_________)

3. I understand in the event of a medical accident, 911 will be called and the EMT staff will determine the medical care and facility. I accept responsibility for any expenses that may be incurred for any illness or injury that may result from my, or my application or participation in BioCoreTek’s projects, including the costs of evacuation, hospitalization, and medical treatment and any sums payable to anyone by reason of any injury or loss of life that I may sustain through my participation in BioCoreTek’s projects. 4. I am physically able to safely complete this Eschalon designated extraction and eradication project. My participation in this project is purely voluntary, no one is forcing me to participate, and I have elected to participate in spite of the risks. I will follow the rules and guidelines for Eschalon level Rapid Response. I am not infected with a level 2 (or higher) pathogen. I am not currently under the influence of alcohol, illegal drugs, or impairing legal drugs. I understand that BioCoreTek may refuse Eschalon designation for its Rapid Response project to any person that its owners, agents, or employees deem a hazard to themselves or to others. BioCoreTek may alter its published or announced requirements for participation in any Eschalon level project and for use of its lab at any time and for any reasons that it may deem appropriate. I agree that should any part of this Agreement be judged invalid by a court with proper jurisdiction that all other parts not so judged shall nevertheless remain valid and in effect. BioCoreTek reserves the right to use voice, video or other photographic images of the Candidate for promotional, educational, or other purpose, and Candidate hereby consent to such use, without compensation. The laws of the Province of Ontario shall govern in this agreement and that the courts with jurisdiction in Mississauga, shall have jurisdiction in any dispute that may arise between Candidate and BioCoreTek. I have read, fully understand, and hereby agree to the terms of this agreement, voluntarily and with knowledge of the Project and its risks. I acknowledge that this agreement shall be effective and binding upon myself, my heirs, assigns, personal representatives, and estates.(__________)
